Sign up for our two timed races and compete to win a grand prize, given to the individuals with the best overall times in all the races! Run the 2008 Conroy Orthopedic and Sports Physical Therapy Summer Circuit for just $27 for residents, $32 for non-residents. Non-refundable fee includes: registration, t-shirts and day-of-race entertainment.

By signing this form you may be giving up certain rights, before you sign read and understand this form. Authorization, Certification, wavier and release. The undersigned for and in consideration of being given the opportunity to participate in the 2008 Summer Circuit 5K Runs and/or Walks (Event), to be held throughout 2008, hereby releases and waives any and all claims for himself, his heirs, dependents and assigns, against the H-F Racquet and Fitness Club, the Illinois Association of USTA Trac and Field, and other participating agencies, and all their officers, employees, agents volunteers and sponsors, from any and all liability whatsoever for any injuries, damages and claims that may arise out of this participation. I fully recognize that a risk of injury exists. I assume all risk. I certify that I am physically fit and will have trained sufficiently for the event. I also grant permission to the H-F Racquet and Fitness Club and its authorized agents to use my name and photograph, video tapes, motion pictures, recording and any other public relations account of the event for any purpose, including commercial advertising without monetary payment.
